Service Description
Evaluation and treatment of acute concerns, preventive care, wellness support, and routine primary care services provided through secure telehealth visits with a licensed nurse practitioner. For chronic disease management or ongoing prescription therapy, periodic laboratory evaluations may be required at the provider’s clinical discretion to support safe and effective treatment.
Cancellation Policy
Payment is required in full at the time of booking to reserve your appointment. Appointments canceled or rescheduled with less than 24 hours’ notice may be subject to a cancellation fee or may be non-refundable. Patients may book appointments online up to 2 hours prior to the scheduled appointment time, based on availability. No-show appointments may be non-refundable. By scheduling an appointment, you acknowledge and agree to these terms.
