Service Description
Follow-up appointment for ongoing management of weight loss, primary care, and wellness goals. Includes review of symptoms, medications, treatment progress, laboratory results when applicable, and treatment plan adjustments as needed.
Cancellation Policy
Payment is required in full at the time of booking to reserve your appointment. Appointments canceled or rescheduled with less than 24 hours’ notice may be subject to a cancellation fee or may be non-refundable. Patients may book appointments online up to 2 hours prior to the scheduled appointment time, based on availability. No-show appointments may be non-refundable. By scheduling an appointment, you acknowledge and agree to these terms.
