During this follow-up visit, we'll review your symptoms, medications, chronic health conditions, laboratory results when appropriate, and any changes in your overall health. Your treatment plan will be updated as needed, with a focus on preventive care, chronic disease management, and helping you achieve your long-term health goals.
Cancellation Policy
Payment or an active membership plan, as applicable, is required to reserve your appointment. Appointments canceled or rescheduled with less than 24 hours' notice may be subject to a cancellation fee or may be non-refundable. Patients may book appointments online up to 12 hours prior to the scheduled appointment time, based on availability. No-show appointments may be non-refundable. By scheduling an appointment, you acknowledge and agree to these terms.
